Family Connections Program

We will be your eyes, ears and hands to guide the care and
advocate for your loved one when you can’t be there.

Harbor Home Healthcare is introducing a new comprehensive program to care for your loved one. Our team of nurses and companions are specifically trained to be your eyes and ears in a loved one’s home. We provide home assessment to help ensure the safety and well-being of our clients.

Our companions will assist with personal care, errands, exercise, meal preparation, household tasks, cleaning and other activities that are necessary to run a household. Our RNs will accompany your loved one to a physician’s appointment, manage medications and coordinate care with other health care professionals.

If there are other tasks that need attention, just ask and we will help to get the job done! A registered nurse will supervise and oversee each and every client. Communication with the family will be done by your RN on a regular basis in the manner that works for your family. Rates will be guaranteed for twelve months for the plan that is chosen. Payments can be made on a schedule that works best for your family.

Travel Connections

Harbor Home Healthcare offers a travel option for families and loved ones who live apart. Our RNs and companions will accompany your loved one to a family reunion or holiday get-together.

No more concerns about your elderly parent traveling alone. This service can help a loved one relocate to a new home or a seasonal residence. Staff who accompany clients are experienced travelers and highly trained caregivers. They will be the link from home to the travel destination when you can not make the trip.
